Who should use an Orthotic?

Because perfect feet are very rare, almost anyone can benefit from orthotics. They prevent and alleviate many of the common foot, knee, leg, hip and back complaints that cause discomfort in otherwise healthy people.
Candidates for Orthotics include those people who experience the following:

  • One side of the sole of the shoe wears out faster than the other
  • Frequent sprained ankles
  • Chronic heel, knee or lower back pain
  • Sore shins
  • Toes that are not straight
  • Feet that point inwards or outwards when walking (pigeon toe or out toe)
  • General foot pain.

    Human Foot

    The human foot is a complex structure made up of 28 bones held together by soft tissue material — muscles, ligaments and tendons. It is little surprise then that the average person taking 14,000 steps per day often damages the soft tissue supportive structure of the feet. Over time, soft tissues slowly become stretched, and this loss of support can cause abnormal foot function during walking or running. Therefore, knee, hip and lower back problems may be caused by abnormally functioning feet.

    Common Conditions

    Plantar Fasciitis / Heel Spur — is commonly caused by strain being placed on the fascia, which runs between the toes and the heel. This can cause a bony growth at the heel known as a heel spur.

    Symptoms:

    Sharp pain originating from the bottom and/or inside margin of the heel.

    Pain experienced when rising from bed in the morning.
    Pain in the arch/heel when standing following prolonged sitting.

    Solutions:

  • Massage arch with frozen water bottle.
  • Pain relief i.e Panadol
  • Calf stretches
  • Orthotic Therapy to stop plantarfascia from stretching.


    Knee Pain — When our feet roll inwards, the tibia (lower leg) rotates inwards, exerting pressure, twisting the knee cap and damaging the surrounding muscle structure and knee carfilage.

    Symptoms:

  • Pain when walking up stairs.
  • Pain increases with physical activity.
  • Pain is experienced on the inside of the knee.

    Solution:

  • Orthotic Therapy to realign foot and knee.
  • Strengthening exercises for quadricep muscles.


    Back Pain - When the feet roll inwards (excess pronation)this causes the legs to rotate inwards and the the hip to tilt forwards causing an increase in the curvature of the lower back. This results in increased stress on ligaments, cartilage and nerves.

    Solution:

  • Stretching exercises for back.
  • Heat packs to ease muscle stiffness.
  • Orthotic therapy to realign feet, knees and back.
  • Physiotherapy if required.


    Morton’s Neuroma — affects the nerve that runs between the toes. This nerve is responsible tor sensation and feeling on either side of the toe. Pressure and continued irritation may cause: A burning/sharp pain — usually between 2 or3 toes and/or numbness between the toes.

    Solution:

    Foot Mobilization to realign the bones in the feet. This frees the trapped nerve.
  • Orthotic Therapy to stop excess pronation. Excess pronation allows the bones in the foot to unlock and move around, which can cause the metatarsal bones to move and impinge the nerve which runs between them. When the foot is stopped from rolling inwards excessively it enables the bones to remain locked, not moving, and therefore impinging the nerve.


    Bunions — An enlargement of bone or tissue, either at the base of the big toe, or on the outside of the foot. The enlargement is often due to excess pronation of the foot.

    Solution:

  • Orthotic Therapy to lock the bones in the foot thus preventing the big toe from being pushed towards the second toe.
  • Surgery in extreme circumstances due to the enlargement of bone or tissue, either at the base of the big toe, or on the outside of the foot. The enlargement is often due to excess pronation of the foot.


    Achilles Tendonitis — Causes inflammation and degeneration of the Achilles tendon — usually affecting athletes. Pain can be a shooting, burning or aching sensation. The most common cause of Achilles Tendonitis is hyper pronation, which adds stress and exerts torsion on the tendon.
    Some footwear can aggravate Achilles Tendonitis. Shoes that do not have adequate flexibility in the sole and have excessive heel cushioning will worsen the problem.

    Solution:

  • Orthotic therapy to stop the Achilles tendon from being overstretched.
  • Calf stretching exercises.
  • Anti- inflammatories if it is a chronic condition.
  • Physiotherapy if required.


    Claw Toe —Toe deformities are commonly caused by a muscle imbalance in the foot as a result of flat feet, traumatic injury or disease of the toe joints. There are at least 6 sets of muscles that control each toe, each muscle stabilising one of the bones in the toe. If a foot flattens (or pronates’) this can cause a muscle imbalance in the foot leading to the toes clawing.

    Solution:

  • If pronation is the cause of the condition, a Podiatrist can prescribe and fit the patient with customised orthotics to provide better arch support and help aid the muscles in working together.


    Growing Pains — “Growing pains” do not exist! If growing pains were a normal occurrence, then all children would experience it — this is not the case. Growing pains”, are actually a biomechanical anomaly associated with growth spurts and can be treated.


    Pigeon Toe — When a child walks with their feet turned in. This is the result of either the shin bone twisting inwards or tight groin muscles.

    Solution:

    Gait plate orthotic therapy which untwists the shin bone as the child grows, so that the foot ends up facing straight ahead.


    Out Toe — when a child walks with their feet turned outwards. This is a result of either the shin bone twisting outwards or tight buttock muscles.

    Solution:

  • Gait plate orthotic therapy which causes the shin bone to twist inwards as the child grows, so that the foot ends up facing straight ahead.


    Aching Legs - The main cause of tired and aching legs is stress and traction on the calf muscles at the rear of the leg. This is the result of excess pronation (feet rolling inwards) and the resultant internal leg rotation.


    Glossary Of Treatment

    ORTHOTIC THERAPY:- Is used to realign the body. If the body is out of alignment this can cause increased wearing of the ankles, knees, hips and back, causing muscular compensation and pain. Orthotic therapy can help eliminate pain.

    PROLOTHERAPY:- Prolotherapy is a highly effective method of treating chronic pain by strengthening weak ligaments. This is achieved by injecting glucose into these ligaments, to cause an inflammatory reaction so the body can heal itself by making strong healthy tissue.

    ACUPUNCTURE:- Acupuncture is a 5000 year old Chinese art of healing that involves the insertion of very fine, sterile, disposable needles into specific parts of the body. It is used for treatment of pain and dysfunction which is caused by trigger points (irritable spots in the muscle). The needle will enter the trigger point and cause it to release helping reduce pain. Acupuncture is also used to break down scar tissue in the muscle and around tendons and ligaments.

    FOOT MOBILISATIONS: Where orthotic therapy is required, patients will benefit greatly by undertaking foot mobilisation. Foot mobilisation realigns the bones of the foot thus reducing the stretch on ligaments and tissues in the foot.

    PILATES: This is an exercise style that involves training your muscles to improve posture and alignment. By focusing on your core abdominal strength you are better able to support the larger ligaments, tendons and joints and help reduce back pain.

    CT SCAN: A CT scan is a scan using an x ray machine that takes a cross section shot of the body using very little radiation. The purpose is to measure the bone length of the legs to determine if there is a short leg.
